To make a new patient appointment, please follow these steps:
Step 1
Pick your Medical Provider and schedule below.
Step 2
Give us a call at 219-228-4224 and give us your insurance information so we can verify benefits and make sure there won’t be any unexpected costs.
Step 3
Fill out your intake paperwork that is sent to your email right after you request your appointment.
Step 4
Gather any recent blood work that has been done and download it to your patient portal (be sure to click “share” so we can see them), email it to us at mo***************@***oo.com, or bring it with you to your appointment.
Step 5
You will get an email reminder two days before your appointment. You will get a text message reminder one day before your appointment. We can’t wait to meet you and help you along your health journey! See you soon!
