New Patients:
To Make a NEW patient appointment please follow these steps:
1. Pick your Nurse Practitioner and schedule below.
2. Give us a call at 219-228-4224 and give us your insurance information so we can verify benefits and make sure there won’t be any unexpected costs.
3. If you would like to be put on the cancellation list to get in sooner, you can request that when you call with your insurance information.
4. Fill out your intake paperwork that is sent to your email right after you request your appointment.
5. Gather any recent blood work that has been done and download it to your patient portal (be sure to click “share” so we can see them), email it to us at Monarch.FrontDesk@yahoo.com or bring it with you to your appointment.
6. You will get an email reminder 2 days before your appointment. You will get a text message reminder 1 day before your appointment.

*Please note that if intake forms are not completed and insurance is not verified at least 48 hours before your appointment, we reserve the right to cancel your appointment.
NOTE: Due to no-shows and last minute cancellations, we now ask for a credit card to be on file in order to make an appointment. That credit card will never be charged without you being notified.
We require a 24-hour notice for cancellations. If your appointment is not canceled within the 24-hour notice, we reserve the right to charge a $40 no-show fee.

Patient No-Show Policy
Quality care for our patients is our priority. Please take a few minutes to review. If there are any questions or concerns, please let us know.
Definition of a “No-Show”: Monarch Health & Wellness defines a “no-show” appointment as any scheduled appointment in which the patient either:
- Does not arrive to the appointment.
- Cancels with less than 24 hours’ notice.
- Arrives more than 15 minutes late and is consequently unable to be seen.
Impact of a “No-Show” Appointment
“No-Show” appointments have a significant negative impact on our practice and the care we provide to our patients. When a patient “no-shows” a scheduled appointment, Monarch Health & Wellness loses an opportunity to get another patient in who needs care.
How to Avoid Getting a “No-Show”:
- Confirm your appointment.
- Arrive early to your appointment.
- Give at least 24 hours’ notice to cancel appointment.
