New Patients
To Make a NEW patient appointment please follow these steps
Step 1
Pick your Nurse Practitioner and schedule below.
Step 2
Give us a call at 219-228-4224 and give us your insurance information so we can verify benefits and make sure there won’t be any unexpected costs.
Step 3
If you would like to be put on the cancellation list to get in sooner, you can request that when you call with your insurance information.
Step 4
Fill out your intake paperwork that is sent to your email right after you request your appointment.
Step 5
Step 6
You will get an email reminder 2 days before your appointment. You will get a text message reminder 1 day before your appointment.

*Please note that if intake forms are not completed and insurance is not verified at least 48 hours before your appointment, we reserve the right to cancel your appointment.
NOTE: Due to no-shows and last minute cancellations, we now ask for a credit card to be on file in order to make an appointment.
We require a 24-hour notice for cancellations. If your appointment is not canceled within the 24-hour notice, we reserve the right to charge a $40 no-show fee to the credit card on file.

Insurances
We take most insurances for your office visit. If you have insurance we do not take, affordable self-pay options are available. We do not take Medicaid, Ambetter, or CareSource at this time.
